The terms of medical care are established separately when you become a patient and are governed by your patient agreement, applicable consent forms, our Notice of Privacy Practices, and applicable law.

Revity Medical operates clinical locations in Utah providing personalized wellness and longevity services, including hormone replacement therapy, medical weight loss, NAD+ therapy, IV therapy, primary care, and related services. Service availability varies by location and is subject to clinical evaluation.
The Site provides general information about our services and a means to request a consultation. Submission of a consultation request through the Site does not create a patient–provider relationship and does not guarantee that you will be accepted as a patient or that any specific service will be provided.

The content on this Site is for general informational and educational purposes only. It is not medical advice, diagnosis, or treatment, and it is not a substitute for consultation with a qualified healthcare provider.

11. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, REVITY MEDICAL LLC, ITS OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, AND AFFILIATES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES — INCLUDING LOST PROFITS, LOST DATA, OR LOSS OF GOODWILL — ARISING OUT OF OR RELATED TO YOUR USE OF, OR INABILITY TO USE, THE SITE, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
OUR TOTAL CUMULATIVE LIABILITY FOR ANY CLAIMS ARISING FROM OR RELATED TO THE SITE WILL NOT EXCEED ONE HUNDRED DOLLARS ($100) OR THE AMOUNT YOU PAID US, IF ANY, FOR ACCESS TO THE SITE IN THE TWELVE MONTHS PRECEDING THE CLAIM.
This limitation does not apply to claims arising from medical care provided by Revity Medical, which are governed separately by your patient agreement and applicable law. It also does not apply to liability that cannot be excluded or limited under applicable law.
12. Indemnification
You agree to indemnify and hold harmless Revity Medical LLC and its officers, directors, employees, and agents from any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or related to your use of the Site, your violation of these Terms, or your violation of any rights of another party.
14. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of Utah, without regard to its conflict of laws principles. Any dispute arising out of or relating to these Terms or your use of the Site shall be resolved exclusively in the state or federal courts located in Utah, and you consent to the personal jurisdiction of those courts.
This dispute resolution provision applies only to disputes related to use of the Site. Disputes related to medical care are governed separately by your patient agreement and applicable law.
15. Severability
If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that the remainder of these Terms remain in full force and effect.
16. Entire Agreement
These Terms, together with our Privacy Policy, constitute the entire agreement between you and Revity Medical regarding your use of the Site and supersede any prior agreements between you and Revity Medical relating to the Site.
